4. What is your pet's general health? Does it have any special needs that the Inquisition needs to be aware of?

5. Do you have a caretaker appointed for your animal should you become incapacitated, are killed in the line of duty, or disappear through a rift?

5a. If so, who is the appointed guardian?

5b. If not, would you be willing to have the Inquisition make arrangements for your pet under aforementioned circumstances?

6. Does your pet currently serve an official function in the Inquisition?

7. Is your pet trained for use in combat? Can it's species be trained in combat?

7a. If it can but has not undergone training, would you be interested in having it trained?

8. Does your pet have other skills of which the Inquisition should be made aware?
